Part Time Patients

WHAT IS IT?

A part-time patient is someone who is usually just testing the waters of transitioning medically with hormone therapy. They may be shy, short on time, or could have any number of other reasons as to why they do not want the full patient experience. For these people, we have created an alternative track that has a mixture of patient autonomy with minimal interference on our part.

SHORTENED EXPLANATION OF THE PART TIME PROCESS:

- Book a Part Time Patient Appt (costs $100).
- Complete the online medical intake forms emailed out to you.

- Complete the initiation appt.

- Complete the CBC/CMP labs (can submit recent labs if done in last 6 months).

- Prescription for a 90 day supply is sent out to your pharmacy. 

- Every time you need a refill on this, pay a renewal review fee (between $100-$130 depending on type of medication).

*Additional appts cost $100 per occurrence. Additional lab work costs vary depending on if you use insurance or not (runs between $50-$100 per panel through Labcorp in general).

LONGER EXPLANATION OF THE PART TIME PROCESS:

Essentially, you would book the appointment and check the box stating that you want to be a part-time patient. What this will do is let us know that we are going to collect your medical information just like any other patient, do an appointment to go over those same completed forms, and at some point you will submit a basic lab panel consisting of a complete blood count (CBC) and a comprehensive metabolic panel (CMP). All of this is basically the same as any other patient in the beginning. But after these things is when it changes course from where a person seeking a full patient experience would go. 
From here, you would be prescribed a low dose of hormone therapy rather than a full dose. What this means is that the maximum amount of physical changes that you could reach would be those of someone who is perhaps non-binary or androgynous. Or if you choose an even lower dose (depending on your goals), then you may only get extremely mild physical effects but still could experience the mental health benefits of HRT. 

No further requirements are placed on you beyond this as far as follow up appointments and labs. You can always opt to do these things if you would like, but there will be no set requirement for them since the doses you will be on are relatively safe from causing physical harm if your initial health screening reviewed by the provider is passed (as will be covered in detail by the medical intake paperwork and during your initiation appointment). 

If at any point during the review of your medical history, the initiation appt, the lab review, or the renewal reviews you are found to be in less than ideal health for starting/continuing HRT, your fee will be refunded as we would not then be able to take you on as a part time patient.

You will be issued a prescription after completion of the CBC/CMP labs and the initiation appt for a 90 day supply of medication. Each time you need a further 90 day supply, you would then pay a renewal review fee to reauthorize the prescription again. No other costs would be associated with this care, unless you chose to complete additional appointments or labs later on, or decided to become a full time patient.

 MEDICATIONS

As mentioned before, the medications prescribed are only going to be able to be used for low dose (slow) transitioning. So there are some limitations on what can be prescribed. For example, we will not prescribe Estradiol or Testosterone injections for part time patients. The medications used are listed below and can be discussed during your appt. 
+ For Testosterone, we will prescribe topical formulations at half dose (we recommend the generic option for Androgel as it is usually the cheapest). 
+ For Estradiol, we will prescribe patches (up to the dose of 0.1mg) or pills (up to the dose of 2mg twice daily).

*Please note that being a part time patient is a very autonomous (self-driven) experience compared to that of full time patients who have constant support via the clinic app and their appts. This is not included if someone is a part time patient as it is treated more as "episodic care" rather than the continuously monitored and heavily supervised management of full time patients.
