Get started on your individualized transition plan! Appts are completed by Telemedicine Video Chat for your convenience. Rxs are sent electronically to your pharmacy (or can be mail-ordered), and labs can be completed in your local area. A one-time $70 admin fee is due when booking the appt. The cost after that is only $38/month, which begins 30 days after your 1st appt.
